Fall 2008 Registration

Go to HANDOUTS to obtain registration forms.

 NOTE: ALL Travel/Competitive players will need to provide a wallet photo and and a copy of a birth certificate along with the registration form.  Travel Program is now CLOSED

 

Recreational Registrations close on August 29, 2008 

Please complete all forms as required and mail with payment to the PSC PO box.

 

NOTE: All coaches and volunteers are required to complete a disclosure form located in HANDOUTS section.
