Saturday, January 1 Player Injury - Forms and Information What Parents Need to Know... Prior to a Player Injury! The Insurance Policy that is carried by ALL local little leagues is a supplemental coverage policy. Meaning; Claims are submitted first by the parent to the parent's primary insurance carrier (ie: their employer's insurance). Expenses that are not covered by the parent's primary insurance carrier are then submitted to LLB's Insurance carrier (a $50 deductible does apply). What to Do At The Time of a Player Injury... - Remain Calm and Provide Appropriate Aid to the Injured Player - If Emergency Medical Assistance is required - Call 911 immediately... - If a non-emergency, Remember to treat the injury and follow the P-R-I-C-E-S guidelines: - Protection, Rest, Ice, Compression, Elevation, Support. What to Do Immediately After a Player Injury... With the Assistance of the Team Manager/Coach, The Parent/Guardian Should Complete The "Player Accident Notification Form": Instructions on Completing the Accident Notification Form Accident Notification Form. The Injured Player's Team Manager or Coach and/or the player's Parent/Guardian Should Notify PHLL within 24 hours of the Player Injury either by PHONE at 785-3785 or by EMAIL.

Medical Release Little League Baseball requires that all local leagues (PHLL) have a Medical Release Form completed at Registration or prior to the player participating in a game or a practice. These forms are to be carried by the player's "Team Manager" at all practices and games throughout the season.

Wednesday, September 1 Volunteer Application PHLL supports Little League's "Child Protection Program" which requires all local leagues (PHLL) to have on record an annual Volunteer Application form and to conduct a mandatory background check on all local league volunteers including, but not limited to, League Officials, Team Managers, Coaches, Umpires, Scorekeepers, etc...
