Player Injury - Forms and Information
What Parents Need to Know... Prior to a Player Injury!
The Insurance Policy that is carried by ALL local little leagues is a supplemental coverage policy.
Meaning; Claims are submitted first by the parent to the parent's primary insurance carrier (ie: their employer's insurance). Expenses that are not covered by the parent's primary insurance carrier are then submitted to LLB's Insurance carrier (a $50 deductible does apply).

What to Do At The Time of a Player Injury...
- Remain Calm and Provide Appropriate Aid to the Injured Player
- If Emergency Medical Assistance is required - Call 911 immediately...
- If a non-emergency, Remember to treat the injury and follow the P-R-I-C-E-S guidelines:
- Protection, Rest, Ice, Compression, Elevation, Support.

What to Do Immediately After a Player Injury...
With the Assistance of the Team Manager/Coach, The Parent/Guardian Should Complete The "Player Accident Notification Form":

The Injured Player's Team Manager or Coach and/or the player's Parent/Guardian Should Notify PHLL within 24 hours of the Player Injury either by PHONE at 785-3785 or by EMAIL.

Medical Release
Little League Baseball requires that all local leagues (PHLL) have a Medical Release Form completed at Registration or prior to the player participating in a game or a practice. These forms are to be carried by the player's "Team Manager" at all practices and games throughout the season.

Volunteer Application
PHLL supports Little League's "Child Protection Program" which requires all local leagues (PHLL) to have on record an annual Volunteer Application form and to conduct a mandatory background check on all local league volunteers including, but not limited to, League Officials, Team Managers, Coaches, Umpires, Scorekeepers, etc...
