To participate in Little League ALL PLAYERS need to fill out and return a Medical Release Form. Some people have already done this, others have not. If you have not yet filled out the Medical Release form please do so and return it to your coach during the first week of practice. Click the link below, print out the form, fill it out and return it to your coach THIS WEEK.
Little League Headquarters in Williamsport is also requiring a copy of your child's birth certificate be on file with the local league this year. That means Perry Little League will need a COPY of your child's birth certificate. Procedures have been put in place to protect this information.
