Registration Info

Registration is open to players living in the San Carlos, Lake Murray, Mission Trails and Allied Gardens league boundaries with the league age of 12, 13 and 14.   Please bring the following items:
1) Registration Form on the website.
2) Include copies of proof of address (they will not be returned).
3) $75  fee.

Managers and Coaches are needed.  Please email Patriot Baseball with your contact info.  Everyone who works with the players needs to fill out a volunteer application, so bring it to the first event, or you will not be allowed to be on the field with the team.

The forms open with Acrobat Reader.   You need to have the free program installed to open the document.   Open the forms and enter the information and print both (use the printer icon in acrobat instead of the browser, it works better).   You can find other forms in the Forms & Apps section.
Bring the forms with proof of residency (see below) to registration. We will have blank forms at registration, if needed.    You can order a copy of your players birth certificate online at http://www.vitalchek.com/

Things to bring to Registration
Along with the forms, new players need to bring their birth certificate, returning players do not.
You will also need to show Proof of Residency at registration. Please bring three of the following:
1. Valid Driver's License
2. Voter's Registration
3. School records
4. Welfare/child care records
5. Federal records
6. State records
7. Local (municipal) records
8. Support payment records
9. Homeowner or tenant records
10. Utility bills (i.e., gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
11. Financial (loan, credit, investments, etc.) records
12. Insurance documents
13. Medical records
14. Military records
15. Internet, cable or satellite records
16. Vehicle records
Note: Example - Three utility bills (three items from No.10) constitute only ONE document.

Plus the Medical Release Information, i.e., address & phone number of family physician, name and phone number of two emergency contacts, allergies or medical problems.


Player Evaluation

All Junior (13 & 14) Players must attend a Player Evaluation (Try Outs).   You will be given an evaluation time at registration.   If you do not attend an evaluation, the player cannot be drafted by a manager but will be placed on a team by the Player Agent if there is room.

There is an evalution for Senior First Half.
No Evaluation for 15 & 16 year old players or 14 year old players for the 2nd Half of the Senior Season.

The Evaluation consist of batting, fielding grounders & flys and throwing.  All players should come ready to play.  That means bring your glove, bat, helmet, cleats and hat!   Dress like you would for a game without the jersey.   Please check in 20 minutes before your assigned time.

A player who lives in the boundaries for San Carlos, Mission Trails, Allied Gardens and Lake Murray is eligible to play in the Patriot League.   Any player who moves out of a league's boundary area can still play for that league as long as their play is continuous (they can't play a season with one league, then the next in a different one, then go back to the first one).   Our Player Agent is happy to field any eligiblity questions.
