12th Annual Sudbury Panhellenic Soccer Tournament

The 2010 Sudbury Panhellenic Tournament Dates & Registration Information
The Sudbury Panhellenic Tournament will be on July 23, 24, 25 2010.
To be considered for the tournament you must include:
1. Complete the OSA's Team Entry Form and submit it as soon as possible, be sure to save a copy to send in with your team entry fee. The deadline for accepting entries is July 1st, 2010. Teams who apply late will be put on a waiting list.
2. Payment of tournament entry fee is due prior to July 1st. Send in $350.00 cheque or money order along with a copy of your Team Entry Form.
3. Team list of names, jersey numbers, date of birth and O.S.A. numbers will be required at registration.
4. Properly Registered players' books will be required at tournament headquarters.
5. You must have permission to travel from your district and a valid travel form.
6. Maximum roster of 18 players. You are allowed 3 guest players. Temporary registration permit must be provided for guest players at registration.
7. Only registered team officials will be allowed and must provide O.S.A. registration number on the team roster sheet.
8. For teams from outside of Ontario you must supply proof of medical and personal liability insurance for all players and coaching staff.
