2009 Football Roster Spots Still Available
Openings exist on all squads except Mitey-Mite and Pee Wee.
If interested please fill out the registration form and send it to the P.O.
Box address on the form.
Registration Forms
Things Needed for a Complete Registation
- A copy of participants Birth Certificate (if new)
- A wallet size picture-please no larger
- A full year report card from 2008-2009
- Physical on Pop Warner’s Medical Form dated 2009
- Payment in Full, including raffle tickets is required to hold a spot on the team
- Part of the registration fee ($50) is non-refundable
- Raffle tickets will be given out at the start of the season and the drawing will be by jamboree.
- Any refunds must be done in writing and will not be honored after 8/1/2009
- All paperwork should to be received by the registrar by June 15, 2009.
- No child will be allowed to practice until all paperwork is received.
- No equipment will be given until all paperwork is received..
