The Clinic will be held at R.H Stadium  August 10th - August 28th with the exception of August 14th...From 5pm-7pm...

 Make sure all physical forms are in... and all other misc items as well (birth certificate, 4x6 photo, report card)

if you haven't done your child's physical no worries...  Industrial Medical Group 3070 Skyway Drive 805 922-8222 are taking walk ins call for office hours and fee information... Please note "OYFL Physical Forms" can be downloaded from our hand out section... OYFL will not except any other sport physical forms...

Please come prepared with water bottle, gym shoes and work out gear... Please no jewelry of any kind... so for safe keeping please remove all jewelry at home...
