Oregon District 9 Little League Baseball & Softball: Volunteer Form : Volunteer Background Check Form

VOLUNTEER FORM for BACKGROUND CHECKS and a tool that can be used at registration to help folks see what volunteer positions cover ... and help them find where to take on some responsibilities!
Volunteer Background Check Form

Leagues are required to run Background/Sex Offender Register checks on all volunteers (board members, managers, coaches and others who provide regular service to the league and/or have repetitive access to, or contact with players or teams). A fully completed Volunteer Application, including a copy of a government-issued photo i.d., must be turned in and results of the check known before the position is taken.
Beginning in 2010, two forms are available, and each league makes its own determination as to which form is required for their league. It has been said, "I'm entrusting my child to your care, I should be trusting you with all essential information to help keep my child safe."

Any questions specific to the Volunteer form should be directed to your local league president.

  • District-wide Rules, etc. - Due to the high number of interleaguing and inter-league special games events in District 9, we have established district-wide rules which provide consistency for ALL teams, all divisions, all leagues, all levels, all games. This has proven to be most helpful for managers, coaches, players, umpires, and spectators. The rules are revisited each Fall; potential changes discussed, and rules finalized by District Staff. Goal is consistent to have rules to League Presidents for distribution by February meeting.
