2009 Recreation Soccer Sign-Up Information
Sign-up Form Drop Off Dates & Times Wednesday March 4th 6:00 PM – 9:00 PM Saturday March 7th 9:00 AM – 1:00 PM Wednesday March 11th 6:00 PM – 9:00 PM Saturday March 14nd 9:00 AM – 1:00 PM Wednesday March 18th 6:00 PM – 9:00 PM LATE FEES WILL APPLY AFTER 12:00 PM ON MARCH 18TH. NO REGISTRATIONS WILL BE ACCEPTED FOR DIV 4, 3 & 2 ONCE TEAMS ARE DRAFTED IN APRIL. 2 Easy Registrations Steps 1. Print out and complete the soccer registration form below (1 form per child). 2. Bring the completed form with payment to the Danny D’Elia building on one of the above dates/times. If required, also bring birth certificate and/or recent picture (see below). Make checks payable to Borough of Oakland. Children are placed by age and date of birth as follows:
* Grades listed above reflect the grade a player will be going into effective September 2009. Players entering Kindergarten are eligible to play regardless of the Date of Birth. High School Seniors are also eligible. A recent picture is required for Divisions 4, 3 and 2. A copy of your birth certificate is also required for all “new” players. (Applicable if you never played in the Oakland Recreation Soccer Program.)
