Fall Ball 2009 Registration
FALL 2009 REGISTRATION
WILL BE ON THE FOLLOWING DATES:
Registration fee: $65.00 per player
Need to bring Birth Certificate and
3 proofs of residency to registration.
This includes all current and past players, no exceptions!
Paperwork from prior seasons can NOT be used. New copies have to made every season to meet district requirements.
The following is a list of acceptable proof of residencies. You must present three (3) proofs at registration that have your address clearly printed on the document.

Driver’s License

Voter’s Registration

School records
-
Homeowner or tenant records
-
Utility bills (i.e., gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
-
Financial (loan, credit, investments, etc.) records
-
Insurance documents
-
Vehicle records
-
Employment records
-
Military records
-
Note: Example – three (3) utility bills constitutes as only ONE document.
Mark your calendars with these important sign up dates:
Sat 8/22 9am-noon Registration at NWLL (concession stand)
Thurs 8/27 6pm -8pm Registration at NWLL (concession stand)
Sat 8/29 9am-noon Registration at NWLL (concession stand)
Thurs 9/03 6pm-8pm Registration at NWLL (concession stand)
