Use this form to document problems/concerns about issues you have with a specific program, action or ruling. Completed forms should be forwarded to the appropriate Director. uncompleted forms will delay corrective actions.

Grievance Form

This guideline is the process we use in handling and dealing with conflict resolutions. The idea is that there is an outlet in which our members can address issues of concern in an organized manner. This guideline is used for concerns over coaching issues, rules and code of conduct issues. Again this is a guideline, there are some issues that require immediate action, especially if it deals with the health and welfare of our participants. This posting is for informational purposes.
