Medical Release form
Here is the Medical Release form. You can fill it out online, print it, and deliver to the inperson registration, or go with mail in registration.

Age chart
How old is your kid? Here is what Little League says about it! This is the updated age chart for 2009. The minimum age for NSCLL is league age 9. However, the NSCLL allows league age 8s who are in the 3rd grade in the 2008-2009 school year to participate. These players will be classified as league age 9s.
