Good delegater. 4. Willing to do most of your duties PRIOR to the start of the season. 5. Relationship-oriented. Saturday, September 29 Insurance and Medical Forms Each player must have had a physical examination completed within the last year that indicates you are able to compete in athletic competition. You must also turn in an insurance form. The forms are available on the Home Page. You may submit a copy of a prior physical that was completed within the last year. Please turn forms into your coaches or Team Parent. Insurance forms are due by Oct. 10th and Medical Forms ASAP but no later than October 20th. |
