Use these forms to complete the registration from home. Follow these simple steps to save time during the registration process. The forms are formatted in Adobe Acrobat to allow text insertion from your computer.

1. Download the forms, fill out the three required forms from your computer or print them fill out by hand.

2. Bring the forms to one of the walk in registrations along with your registration fees or mail the registration kit to NMLL along with your check.

NOTE!!!! All new players not already on record with NMLL, please don't forget to include a Birth Certificate copy and two forms of address verification for the Little League residency requirement.

NOTE!!!! The Volunteer Application is required to be filled out by all parents even if the only role you will fill is that of a transportation or carpool parent for practice and games.
