Please bring your childs birth certificate, health insurance information and confirmation of USA Hockey registration.

Mini-Mite Rec $250 Squirt Comp $575
Mite Rec $325 Pee Wee Comp $775
Squirt Rec $400 Bantam Comp $875
Pee Wee Rec $400 Midget Minor Comp $875
Bantam Rec $425 Midget Major Comp $950
Girls U19 $350 High School $425
**All fees do not include $50 USA Hockey fee, you can register with USA Hockey online at www.usahockey.com.
Non-registered players are not allowed on the ice for insurance reasons. Players with outstanding fees from last season or who have not paid in full for this season will not be allowed to begin practicing.

NOTE FOR ALL REGISTRANTS:
A copy of the skaters birth certificate, USA Hockey registration and insurance information is required.
Payment is due in full to be registered & placed on a team.
Cash, Check, Mastercard & Visa are accepted.
PARENT OR LEGAL GUARDIAN MUST SIGN ALL FORMS for players that are under the age of 18 - Waivers, Consent, etc.

Registration Forms
If you go to "handouts" on the left hand menu you can download all of the registration forms for the 2007-2008 season. This way you can have them filled out prior to coming to the registration dates. On the NMAHA form please only fill out the form(s) associated with the team(s) you intend to play for(Recreational, Competitive and/or High School)
