Each year we are in need of an updated medical form/physical for each participant. It must be dated within the same calendar year of play. So for this year we would need the medical form signed by the physician after January 2009. Sometimes that becomes problematic due to restrictions by ones insurance company. I wish we could be more accomodating but unfortunately this is not our rule. The medical forms are needed before the first day of practice on August 1st. If you are in need of an alternative to your regular physician, the Wickford Junction Medical Walk-In could help. They will supply a "Sports Physical" for a cost of $35. This is not covered under insurance. I am listing their phone number and hours below. They recommend calling ahead to make sure they are not too busy and also coming at least 2 hours before closing. Also, clearly state what you are there for.
