COJ Continuous service form

This form is to be filled out if you are a returning coach/volunteer. Who have coached or volunteered in 2012 or 2011 with any COJ Parks and Rec approved sport, where you had your background ran through the city

This form is to be filled out and turned in to the association by all coaches / volunteers. That's all who are trying to attain a badge with the association. That's new and returning coaches / volunteers
