Name Listed Alphabetically by LAST NAME
Name should be exactly as it appears on Birth Certificate
3 Proofs of Residence
Original Birth Certificate
Just need to see it this time only should carry a copy with the affidavit package at all time
Little League Medical Release Form - Signed
League Map with Location of Each Player Identified.
