1. Fillout form
2. Deliver or email order form to the Academy. Email:LieselCampbell@aol.com
3. Payment: Pre pay by check, cash or credit card. Order will not be placed until payment is paid in full.
4. Orders must be placed through NFBA only.
5. Please do not call the uniform company, they will not answer your question or place your order.
6. Make sure size is correct...no returns due to wrong size.
7. List your name, age division, jersey number, contact number on order form
