Youth Soccer Month State Proclamations:
Youth Soccer Month is an opportunity for everyone to take advantage of the national attention created from the US Youth Soccer National Office surrounding Youth Soccer Month. For those involved with local government, you can take part by having your city or state officially recognize September as Youth Soccer Month. Check out the celebration by clicking the logo above.

All recertifying referees have had a USER ID and Password created for them by the USSF. Do not use the "new user" link at the US Soccer Information Systems homepage. Please see the information below on how to determine your USER ID and Password.

If you are having trouble logging into the www.ussoccer.info site, please try the following: Your Password was mailed to you.  If you do not know it, click on the "Forgot Password" link. On the following screen, enter your USER ID as given above (R + USSF ID number), and your password will be e-mailed to you at the email address you have on file. If you have not provided a valid e-mail or do not receive the "Forgot Password" e-mail from the USSF, please call Customer Service at 312-528-1214 and they can provide it for you.

THIS COMPLETED FORM MUST BE RECEIVED AT THE ADDRESS OR FAX NUMBER BELOW PRIOR TO TRAVEL AND/OR COMPETING AS A GUEST PLAYER!
