To facilitate the registration process, parents are requested to register online FIRST by going to this website: https://www.nhyfsc.org/registration
scroll down and select the NASHUA ELKS CRUSADERS association, and completing the conference registration packet. Parents of each participant MUST complete the online registration. You will have the chance to print the completed NHYFSC FRONT and BACK Forms which need to be initialed and signed for registration. You can also print the NHYFSC medical clearance form. YOU MUST PRINT THESE BOTH AND BRING THEM TO REGISTRATION

In addition to these forms, you will need to complete and submit the following forms:

1. NHYFSC Medical Clearance Form (or physical form) signed by the physician. This form must be signed by the physician this year and the physical must be performed no sooner than last year.

2. Medical Release Form. This form includes your emergency contact and insurance information and must be fully completed.

3. Documentation Acknowledgement Form. This form covers several other informational documents. It is your responsibility to read and comply with these documents.

4. A legible copy of the participant's birth certificate (new members only).

5. Two copies of the FINAL report card (front and back) received in June. The report card will be mailed to the respective registrar upon receipt.

Spirit and Football Cost Due at Registration: $150.00 for the first participant, $75.00
for each additional participant in the same family, $50.00 for Spirit Mascots,$100.00 flag participants
plus a $40.00 Uniform Deposit per participant. A fundraising buyout will be
