Individual Registrations due for Fall 2009 Teams:
GVSA Team Roster form for each team
GVSA Player/Coach Registration Forms Required for at least:
U8-U10 8 players; U11-U12 10 players; U13+ 13 players - Players who have not previously played in GVSA require a Legal proof of Age and Identity (e.g. Copy of Birth Certificate). Hospital Birth Certificates and Baptismal Certificates are not acceptable.
One Coach 18 years old or older
Unsigned forms and players missing Birth Certificates will be considered late
Teams with insufficient players or no coach may be dropped by GVSA!
