Mt. Olive Hockey: Welcome
2009 MOYH Registration Process:
1) Register with USA Hockey - all players must be registered with USA Hockey for rostering and insurance purposes. All personal and contact information for your player is transmitted to our software, so please be sure to enter complete, accurate information. The USA Hockey fee is not part of your MOYH registration.
· Go to http://www.usahockeyregistration.com/ and click “Ice Players & Coaches”
· Check the box to confirm you are over 18 and click “Continue”
· On the Season Validation screen, click the “Continue” button
· Choose the person you are registering – probably “A Child Family Member (under 18)” and click “Continue”
· Fill out the requested information on the form. Please be thorough – this information will be downloaded to our software and will be the source of our contact information for you (both sets of parent’s info, email addresses & phone numbers, etc.). Click “Continue to Checkout”
· Confirm your information and click “Continue” (you can register a second player at this time too – it saves the basic information)
· Enter your credit card info and click “Pay Now”
· Click “Open & Print” – this is the confirmation that we need. Please save it to your computer.
2) Send in your USA Hockey Confirmation - email it to Dave Jeska(dmjeska@yahoo.com). Also, please print a copy to be sent with your check (see below)
3) Mail your Deposit Check - Please mail the hard copy of your USA Hockey confirmation and your $400 deposit check to:
