CJSA EMPLOYEE/VOLUNTEER DISCLOSURE FORMNew Registration Requirement for Adult Volunteers
As part of the new requirements of the CJSA, all adult volunteers are required to complete the Volunteer Disclosure Form. All MSC volunteers must complete this form in order to work with children in our program. This includes Coaches, Board of Directors members and any other persons, volunteers or hired workers, who provide regular service to the league and/or have repetitive access to, or contact with, players and/or participants. The purpose of these background checks is, first and foremost, to protect the children. You will find this document under by clicking the above link and accessing the CJSA site.
