REGISTRATION INFORMATION FOR 2009 Season
Parents registering football players should be aware that they will need to provide the following paperwork before your registration is complete. Players will not be given equipment or allowed to begin practice if paperwork is not complete. Some paperwork will be collected in March, and some in July - and everything will be posted here.
New Players - Original Birth Certificate* AND a Copy of Birth Certificate
Returning Players - Copy of Birth Certificate
All Players - Copy of a physical OR a doctors note stating that the child is fit to participate in sports. This needs to be DATED AFTER 1/1/09.
Parents, please keep this in mind as you make appointments or see the doctor.
Please DO NOT wait until the last minute.
All Players - Report Card - A copy of the FINAL 2009 Report Card - obviously you won't have this until June 2009, but keep in mind that you will have to make a copy of all pages of the report card showing all terms - including specials. If your child qualifies as an Academic Scholar, you will need to provide a copy of ALL the terms of the report card (QMS report cards only show 1 term per report... please make and keep a copy of each term!!!)
All Players - Parents will also need to provide emergency medical information / emergency contacts, etc. as well as sign code of conduct, volunteer forms, etc. so be sure to bring your insurance card, doctor name & telephone number, and emergency contact info to registration.
Parents - Please be prepared to volunteer. We have one of the largest Pop Warner programs in the State with approximately 400 children participating. There are many opportunities and they are listed on the Volunteer Info page of this site. Opportunities range from head coach, which requires hundreds of hours, to help at the Hornet's Nest for an hour or two. If you volunteer to coach in any capacity, you will need to fill out a CORI form. Our program depends entirely on, and can only be successful with, volunteers. THANK YOU in advance because every little bit helps!
* New Players - Every year, there are questions as to why parents need to provide Original Birth Certificates. The Hockomock League, which we are a part of, certifies each and every player every single year. The League needs to ensure that players are the correct age to play in the correct division. Age/Weight limits are strictly enforced for safety reasons. Your OBC will be returned at the end of the season when the child has returned his/her equipment.
