These should be all the forms needed to register. The forms that say YS or Youth Services MUST go to youth services, Team registration form and the YS Verification forms are the only two you need to bring to TEAM registration.

The Adobe (.pdf) files and the Word (.doc) files are same, you need an adobe reader (free download) to read/access the Adobe files.
