Parents Agreement

1. I hereby certify that _________________is in good health and capable of participating safely in the Morgan Utah recreation program and has accident and health insurance.

2. I herby authorize the Directors and or coaches of the Morgan Utah recreation program to act in my behalf in accordance with their best judgment in case of an emergency.

3. I understand that there are risks involved with playing recreation sports involving a ball. Serious injury or even death could occur. I certify that the Morgan recreation department is not liable for any accident while participation in the Morgan Utah recreation program.
