In this section you will find Registration and Medical Release forms. If you are registering your child, download both forms, print them out and fill them in. Please bring your completed forms to the scheduled registrations or mail to: Moosup Little League P.O. Box 774 Moosup, CT 06354 by the last scheduled registration date posted on the home page. Please include the registration fee of $55 and a copy of the child's birth certificate if he/she is new to the league. Email moosuplittleleague@gmail.com for further information. Any registrations completed after the posted date will include a $10-15 late fee.

All MLL volunteers must submit a Volunteer Application and provide a copy of their driver's license.

Additionally, any person wishing to be considered as a manager or coach in any MLL division, must also complete a Manager and Coach Application. To be considered for a position, all applications must be submitted by the last registration date.
You can download this form, print it out and fill it in. Forms can be submitted during our scheduled Registration sessions, or mailed to MLL, PO Box 774, Moosup, CT 06354. Forms must be received by the last registration date. A background check will be conducted by MLL officials, in accordance with LLI guidelines.
