IMPORTANT NOTICE
If you register online you will still be required to appear in person to the late registration session listed below in order to provide a copy of your child's birth certificate, proofs of residence, to sign the Registration/Medical Release form and to be given your child's tryout dates and times (Upper Divisions only).
Your child's registration will not be finalized without address and age verification. Links for all forms and handouts are provided in the Registration Packet section below.

- Registration/Medical Release Form
(A pre-filled form will be provided for your signature at in-person registrations when using Online Registration) -
- Please complete all requested information.
- This form can be dowloaded and completed electronically using MicroSoft Word.
- A signed copy must be brought to registration along with
- Original birth certificate
- Two types of proof of residence.

- Volunteer Application Form

- If you would like to help the league please bring a completed copy of this form to one of the in-person registrations. The form can be filled out electronically and printed or you can print out a blank copy and fill it out manually. One of our volunteers will be collecting these forms and will be able to speak to you in more detail about volunteering. You can also visit our Volunteer page on the website for more information.
