2009 PLAYER REGISTRATION FORM

Every player needs to fill out a registration form. To save time at registration, fill out, print and bring it along with you to registration.

2009 MEDICAL RELEASE FORM

A Medical Release Form must be on file with Mission Trails Little League for each player. This form is due at registration. You can download it here, print it, fill it out, and save yourself some time and aggravation at registration.

NOTE: The Medical Release Form is a PDF (Portable Document Format) file and uses the Adobe Acrobat Reader to display. The software package is free! If you need to install the reader, click here. When the page opens, go to the bottom and fill out steps 1 and 2, then click on Download in step 3.

VOLUNTEER APPLICATION 2009

As required by the Little League Child Protection Program, it is now mandatory that EVERYONE who has repetitive contact with the players must submit a volunteer application.

All Board members, managers, coaches, or any other volunteers must turn in this completed form before they will be allowed to work with the kids. These forms will be available at registration, but you can save yourself time by printing it out and having it ready.

Injury Report Form

Use this form if any player or coach is hurt. Also, let the safety officer know of the injury. You can this by contacting her directly or contact the division coordinator.
