Every adult MCLL volunteer, whether serving in an official or unofficial capacity, is required to fill out this form and submit it to league officials.

IMPORTANT: Volunteers must also submit a Request for Criminal History.

2010 Volunteer Application

This form is used for insurance purposes. It must be completed by parents and a league official to notify Little League Headquarters of an accident involving a Little League player. The completed form must be forwarded to Little League Headquarters within 20 days after the accident.

Accident Notification Form

This form is used to report safety hazards, unsafe practices, and/or to contirbute positive ideas in order to improve league safety. This form is to be filled out by a Little League official (such as a manager, coach, umpire, or safety officer) to report a player injury.

Incident-Injury Tracking Report

Use this form to request a reimbursement for items purchased for Mill Creek Little League. Be sure to get the appropriate signatures before submitting the form to the Treasurer.
