The only form required for player registration to be complete is the "Registration" form.

VOLUNTEER APPLICATION
In order to process a volunteer application, which is required for all those that wish to coach or manage, we need that form completed, along with a copy of a drivers license/state issued photo ID.

PLEASE NOTE THAT THE FEE FOR VOLUNTEERS THIS YEAR IS $55. VOLUNTEER APPLICATIONS CANNOT BE PROCESSED UNTIL THIS FEE IS PAID.
