USA Hockey Inline Insurance Coverage
As a sanctioned member of USA Hockey Inline, our registered players are provided with general liability, excess accident and catastrophic insurance coverage. The following attachments provide a summary view and detailed view of the insurance coverage provided to your child through his/her registration with USA Hockey Inline. In addition, should you find a need to file a claim, the attached provides a detailed procedure along with the required forms to do the same.
A recent change in their policy requires that you call K&K Insurance at 800-237-2917 ext. 5623 immediately to report a claim - they will take your injury information over the phone. Once your membership has been verified with USA Inline Hockey (please note our USAHIL Sanction #NJI0032), a claim form will be sent to you. We also recommend that you document the injury using the attached injury reporting form. Remember, this is an excess insurance policy and as such, you must file with your primary insurance company first.

Code of Conduct
Core Values
MIH League has adopted seven core values, which are spelled out in the USA Hockey Annual Guide. The seven core values are:
1. Sportsmanship: the most important value is learning a sense of fair play. Foster friendships with teammates and opponents alike.
2. Respect for the Individual: treat all others as you expect to be treated.
3. Integrity: this includes fostering honesty and fair play beyond mere strict interpretation of the rules.
4. Pursuit of Excellence at the Individual, Team and Organizational Levels: all members of the organization should seek to perform each aspect of the game to the highest level of his or her ability.
5. Enjoyment: it is important for the hockey experience to be fun, satisfying and rewarding to all participants.
6. Loyalty: MIH League aspires to teach loyalty to the ideals and fellow members of the sport of hockey.
7. Teamwork: we value the strength of learning to work together. The use of teamwork is reinforced and rewarded by success in the hockey experience.
Codes of Conduct
In addition to its core values, USA Hockey also has established a set of Codes of Conduct which the MIH League has also adopted. These are designed to guide the organization’s members in their planning, programming and play, not only now but in the future.

The Complete Guide to Hockey Equipment & Maintenance
Although MIH is a non-checking league, each player must have the proper protective equipment in order to ensure his/her safety. The required equipment includes a helmet with full face cage and mouthpiece, elbow pads, hockey shin guards, hockey gloves, inline hockey skates, hockey stick with fiberglass blade and hockey pants. We also recommend that each player wear an inline hockey girdle and shoulder / chest protector. This equipment can be purchased at Consignment Sports (Rt. 9 in Howell), Sports Central (Brick) or the Sports Authority.
The most important part of your purchase is the proper sizing the equipment so it prevents rather than causes injury to your child.
