Our goal this year is to have 100 percent of the safety plans submitted and approved by March 13, 2012 so we can obtain the District Administrator incentive. Approved Little League Form. Parents of player must initiate and notarize the form. This is to be used when a player wants to join your league and does not live within your boundaries. This must be done on a yearly basis. Reminder: that player will not be eligible for all-star play.
