This is a form MGSA asks all parents read and sign. It is meant to represent a commitment by the parent to encourage their daughters to learn good sportsmanship and self-discipline.
You may apply to coach online. Anyone wishing to be on the field or in teh dugout must apply and submit to a background check. A $5 fee will be charged to your credit card to cover the expense of performing the criminal background check.
If you cannot apply online this form must be completely filled out and returned with the $5 background check fee to the team's head coach before you will be allowed to assist.
