Placement on a team is on a first come first serve basis. All participants must be between the ages of 7-15 as of July 31, 2008.
Registration Fees
1st child in program $140.00 (3 Payments of $47.00)
2 children in program $280.00 (3 Payments of $93.00)
3 children in program $325.00 (3 Payments of $108.00)
4 or more children in program $400.00 (4 Payments of $100.00)
**Payment plans are available upon request. Please contact our Treasurer, Tracy Hannum, to set up a payment plan prior to August 1st**
Mandatory Registration Paperwork
1. copy of VALID BIRTH CERTIFICATE
2. copy of 2007-2008 REPORT CARD (MUST HAVE FRONT AND BACK OF REPORT CARD)
3. copy of PROOF OF PHYSICAL EXAM and Signed Doctors Release Form dated 2008
All paperwork is due no later than July 28, 2008. Player/Cheerleader will not be able to practice until all paperwork has been received.
