Please make sure that you print both pages of the physical form.We were just informed that any of your participants that are wanting to have their physicals performed at Urgent Care, will only be able to do so only on Fridays 12-9 and Saturday and Sunday 9-9. Those are the only days that the Dr. at that clinic has available.
