OD Responsibilities

Here are the forms you need! A registration form and medical release form must be completed for EACH player. A parental conduct pledge must be signed for each family. Save some time - download and complete the forms before you come to register! You can also register online or by mail!

Please note that online registration closes FRIDAY, AUGUST 29.

If you register by mail, your forms MUST be received by August 29. Just download and complete all of the necessary forms and mail them in with your registration fee. If you are new to Lower Perk, please be sure to include proof of residency and age. You will need to submit:

- a registration form
- medical release form
- Parental Conduct Pledge
- Proof of residency and age (if new to Lower Perk)
- Applicable registration fee
