Volunteer Application Forms
Help us help your kids!> All volunteers must complete the Volunteer Application Form at the time of registration. Please note that background checks will be run for candidates for select positions (including Directors, Managers, and Coaches) using Choice Point. Lower Perk will cover the costs of the mandatory background check. (Volunteer forms only need to be completed once per year; Spring volunteers do not need to complete these forms for Fall Ball.)
