You can download, print and complete the forms below and mail them to
the address on the form.
The first form listed explains how to complete registration for your child's participation in the upcoming season. Please print and complete the necessary forms for your child and mail to address listed on the form.

For all High School players (both boys and girls) you will still need to complete these forms.
NEW THIS YEAR! US Lacrosse membership fees will NOT be included in your registration fees. It is the responsibility of all players to obtain their membership. You will not be able to play or practice unless you are a member of US Lacrosse.
