All families are asked to submit a coaching evaluation form by using the links below.
Online Evaluation (Survey Monkey)
The online evaluation form is submitted securely and directly to the LSMHA Coaching and Development Co-ordinator.

Offline Evaluation
This form can be downloaded by clicking on the Documents link in the menu on the left, you will be taken to our Documents section where you can download the "Coaching & Program Evaluation" form.
If choosing this option please return it to your team manager or any LSMHA board member.

The Lord Selkirk Steelers Red Minor Atom A2 team capped off a very successful season by defeating Phoenix 2 games to one in the best of 3 final.
