EARLY REGISTRATION:
November 21, 2009, from 9:00 am-1:00 pm 
Early Registration Fee: "WOW" ONLY $85 and for each additional sibling $40

2ND REGISTRATION:
Thursday, December 3, 2009, from 5:30 pm - 8:00 pm
Saturday, December 5, 2009, from 9:00 am - 1:00 pm
2nd Registration Fee: $100 and for each additional sibling $50

3RD REGISTRATION:
Tuesday, January 5, 2010, from 5:30 pm - 8:00 pm
3rd Registration Fee: $125 and for each additional sibling $60

YOU MUST BRING

• Child's Birth Certificate (original, not copy or abstract)
• Immunization Records
• Proof of Residency (3 required)
  • Driver’s license
  • Voter’s registration
  • School records
  • Welfare/child care records
  • Federal records
  • State Records
  • Local (municipal) records
  • Support payment records
  • Homeowner or tenant records
  • Utility bills (i.e. gas, electric, water/sewer, phone, mobile phone, heating, waste disposal).  ONLY ONE TYPE OF UTILITY BILL CAN BE ACCEPTED
  • Financial (loan, credit, investments, etc) records
  • Insurance documents
  • Medical records
  • Military records
  • Internet, cable or satellite records
  • Vehicle records
  • Employment records

Your child's league age is determined by how old they are as of April 30, 2010 and registration approval is based on the District 41 Lakeside boundaries.

Manager and coaching positions are available. Please be sure to sign up at Registration if you are interested in a team. (subject to Board approval)

All Registration and Tryouts will be conducted at Lakeside National Little League Baseball Park.
