Manager_Coach_Pledge

This form is required to be completed for all players. Completed forms should be carried by the Team Manager at all practices and games.

Medical Release Form

This section contains the current Supplemental Rules, late changes added during the season, and the rules specific to the division playoffs.

Supplemental Rules 2005

The following two forms should be completed by the Manager/Coach whenever any player is injured during practice or league play.   The Insurance Claim Form should be completed with as much information as is known at the time of the injury. The completed forms should be forwarded to the Vice President of Administration of the Lincroft Little League at:

and an email should be sent to info@lincroftlittleleague.org reporting the injury.

If a claim is eventually made against our insurance, the Insurance Claim Form needs to be resubmitted with all of the required information and receipts.
