The following two forms should be completed by the Manager/Coach whenever any player is injured during practice or league play. The Insurance Claim Form should be completed with as much information as is known at the time of the injury. The completed forms should be forwarded to the Vice President of Administration of the Lincroft Little League at:

Lincroft, NJ 07738
and an email should be sent to info@lincroftlittleleague.org reporting the injury.
If a claim is eventually made against our insurance, the Insurance Claim Form needs to be resubmitted with all of the required information and receipts.
