Each student Athlete must complete this form and have it signed by their parent/Guardian. The physical form must be completed and signed by a Doctor upon completion of the physical. Get this done as soon as you are able and you will be cleared for all sports for the 2011-12 season at Leland

TDap Boosters must be into the district office no later than June 30th. If this is not done, student athletes will NOT be able to particpate in the summer program. This downloadable handout will give you all the info on how to get this done easily.

If you are an incoming freshmen and have not completed one of these forms, please download and complete. Hand this completed form to Coach Carrozzo (Varsity), Coach Beacraft (JV), or Coach Reed (Freshmen) so we have all the necessary contact info for you.
