Sign Up Forms (Mandatory - Please review note below)

Seasonal sign up forms are found here. Each player, parent/guardian is required to complete the following forms: “Registration form”, “Player Code of Conduct”, “Parent Code of Conduct”, and “Medical Release Form”. These forms will be collected at signups, tryouts, or can be mailed to Leicester Little League, PO Box 393, Leicester Ma, 01524. Coaching and other volunteer positions will require a completed “Letter of Intent” and “CORI” as well. LOI for manager positions must be received by 12/31/10 for consideration.
