LEBL Registration Instructions:


1.  Complete the "2011 Team Submission Form" to submit your team for consideration into the League.
	This is due by October 24th along with full payment of $750 per team.  But send this form in early to get your team in.

2.  Complete the "2010 LEBL Roster Form" by October 24th.  A roster form is required by Oct 24th for use to schedule games.
	Final rosters must include jersey numbers and they must match the jersey numbers used in games.
	Final rosters and jersey numbers are due by November 17th.  Rosters are submitted to the insurance company prior to season starting.

3.  Send forms to the following address:

4.  Each player's parent or guardian must complete a "2010 Player Registration" form.  The head coach must retain these signed forms and have by the November coaches meeting to receive your schedule.  NOTE:  Coaches will retain these medical forms and keep them in their possesion for all practices and games (insurance requirement).  DO NOT SUBMIT YOUR PLAYER REGISTRATION FORMS TO THE LEAGUE.

Note:

- Two team limit per city.
- Maximum 4 players (up to 3 is higly preferred) of a younger age may play up on a older division roster.  A minimum of 6 players must be "of the age" in the division.
- A player may only play on (and be rostered) on ONE LEBL team, including players "playing up".
