Beginning with the 2005-2006 hockey season, there is now a nominal $15.00 surcharge for obtaining a Police Check, however the police check is admissable in all volunteer areas and with all volunteer associations. You will require an LMHA authorization form signed by LMHA before going to the Police Station.
The police check is "good" for 2 years. If you continue to coach for a third year there is a D1 waiver that you sign, which is provided at the annual coaches meeting every September before the start of the season, that allows an extention for that year of your existing police check. The same applies for a 4th year, in which a D2 waiver is needed.  At the 5th year, you must obtain a brand new Police check.

Once your receive your police clearance it must be brought back to LMHA for their records and approval to volunteer as per LMHA rules and regs.

For further information you may also contact the Hamilton Police Services Records Department at (905) 546-4980 and they will give you instructions to obtain a police check.
