This form is required to process background checks on all Little League Volunteers who will be working with the children, including Managers, Coaches, Umpires, Board Members, and other volunteers as deemed appropriate by the Board of Directors. If requested, please complete the application and submit through your manager with a copy of your driver's license or other form of photo identification. Thanks!
