You may want to print the following instuctions before proceeding
Click on the "REGISTRATION" menu item on the left side of the screen.
Click "ON-LINE REGISTRATION - CLICK HERE"
Click on the Season you would like to Register your child for.
Enter your last name and click "SUBMIT"
When you see your name come up click on the “THIS IS ME” button to the right of your name. If your name does not come up click on "CREATE NEW ACCOUNT" and enter all information. Please include a current email address as this is an excellent way for LVTB Managers and Coaches to communicate information to you.
Please read the Liability Waiver and check the two boxes below the Waiver text. Enter your name then click "PROCEED".
Find the players name and click on the “REGISTER” button to the right of their name. If they are not listed, click on the “ADD CHILD” button and enter all information about the child.
Complete the Registration form (only eligible leagues will be displayed) then click on "REGISTER" at the end of the page.
Click on “I am Done Registering My Children…Proceed”
Answer Questions then click “PROCEED”
If Paying by Check STOP HERE. Your registration will be saved but not processed. Mail a check payable to LVTB to PO Box 123, Lake Villa IL 60046. Write the players name, Date of Birth and League on the check. Once your check is received an LVTB representative will go in and enter the check #. You will receive an email stating that your registration has been processed.
If paying with a credit card Click on the “Proceed to Checkout with Via or Mastercard” button. Enter all information requested then Click on “Continue to Secure Payment form”
Enter Credit Card info then click on “I Authorize This Transaction”
You will receive an email confirming your child(s) registration and payment has been processed.
