GVSA Select Registration Form

Medical Release Form: This form is required to be completed by every player. A new form is required each year and MUST be notarized PRIOR to submission at registration. Premier-level players MUST have a properly completed and notarized form AT TRYOUTS to submit in the event they are invited to join a Lakeshore Premier SC Team. In the event a player is not selected, they will still be able to utilize the form when joining another team.

Premier Player Contract: Bring the completed Player Registration form/contract to Tryouts unsigned. If you are invited to join a team, you will be expected to sign this form. This form binds a player to Lakeshore Premier SC for the seasonal year which begins upon submission and concludes when the spring season ends.   When a player is invited to join a Lakeshore Premier SC Team, they are expected to submit the form. Failure to do so may result in the roster position being offered to another player. Lakeshore Premier SC is under no obligation to hold a roster position for any player unwilling to submit the appropriate forms and payment when invited to join a team.
